  • Nursing priorities to care plan for Heart Failure
  • Symptoms of significance, to include how they may differ in males and females
  • Critical Timing and treatments for AMI.
  • Care plan activity intolerance levels per patient with AMI
  • Managing risk for excess fluid volume.
  • Research-based review of the co-morbidities with Pulmonary Disease.
  • Symptoms most critical to assess and monitor throughout acute and post-acute stay.
  • Systems critical to managing the ineffective airway clearance.
  • Nursing priorities to prevent complications
  • Research based recommendations to decrease infection risk, following joint replacements
  • Address impaired physical mobility and maintain affected joint in prescribed position and body in alignment
  • Manage risks for peripheral neurovascular dysfunction

This dynamic course will include statistics which depict the growing complexity of our current and future, to include diagnostic categories with the highest hospital readmission rates.  The session will then focus on sharing tools and innovative strategies for success in the post-acute setting.  

These proactive and comprehensive monitoring systems are developed to prevent exacerbations and hospital readmissions.  The tools and resources offered in this session will be most helpful to assist all levels of staff for the skilled nursing facilities, assisted living and residential care facilities.

This course is intended to instruct the professional on tools and stradgies to reduce hospital readmission.

Professional Objectives:

  1. List 3 strategies to reduce readmissions of residents with Heart Failure (HF). 
  2. List 3 strategies to reduce readmissions of residents with Acute Myocardial Infarction (AMI).
  3. List 3 strategies to reduce readmissions of residents with Pulmonary Disease (e.g., COPD & Pneumonia).
  4. List 3 strategies to reduce readmissions of residents with joint replacements (e.g., Total Hip Arthroplasty-THA & Total Knee Arthroplasty-TKA
  5. Describe how communication and monitoring programs can be effective to prevent all hospital readmissions.

Criteria for Completion:

A score of 70% or more is considered passing.  Scores of less than 70% indicate a failure to understand the material and the test will need to be taken again until a passing score has been achieved.
