List 3 strategies to reduce readmissions of residents with Heart Failure (HF).
List 3 strategies to reduce readmissions of residents with Acute Myocardial Infarction (AMI).
List 3 strategies to reduce readmissions of residents with Pulmonary Disease (e.g., COPD & Pneumonia).
List 3 strategies to reduce readmissions of residents with joint replacements (e.g., Total Hip Arthroplasty-THA & Total Knee Arthroplasty-TKA
Describe how communication and monitoring programs can be effective to prevent all hospital readmissions.

Criteria for Completion:

A score of 70% or more is considered passing. Scores of less than 70% indicate a failure to understand the material and the test will need to be taken again until a passing score has been achieved.
