A variety of organizations will be discussed, as well as initiatives designed to promote healthy outcomes that address social determinants of health.  Insight into community components will be shared and students will identify tools to unveil patient services within their community.  Students will learn about CMS initiatives to coordinate patient care with outside organizations as healthcare providers collaborate with social services to promote healthy communities.

  9. Recognize guiding principles for community leaders engaged in building community efforts to maintain health
  10. Recognize the Community Health Needs Assessment as a tool to measure health indicators and deliverables from the system to the community
