Hour 1

  • Medical history
  • Medication and CAM use
  • Post extubation dysphagia
  • NG/PEG use
  • Respiratory muscle weakness
  • Dentition
  • Any prior procedures or instrumental assessments completed

Hour 2

  • Determine questions to ask the patient based on the chart review
  • Relate disorders/disease processes to specific muscles that would be affected including the impact of lingual sarcopenia

Hour 3

  • COVID-19 impact on dysphagia evaluations
  • Protocols to put in place during COVID-19
  • Identify aerosol generating procedures/screens
  • Assessments vs. non aerosol generating procedures
  • Discussion of screens that are aerosol generating vs. non aerosol generating
  • Screens appropriate for nursing
  • Dysphagia assessments

Hour 4

  • Identify the deficits observed in the assessment
  • Determine need for any further testing/instrumental studies and appropriate referrals
  • Determine most appropriate/safest food and liquid consistency on the IDDSI
  • Non-compliance statistics
  • Identify appropriate compensatory strategies for patient

Hour  5

  • Family and staff education
  • Education on appropriate feeding techniques
  • Discussing how to work with the patient and/or family that chooses to opt out of the recommendations including the use of EDAR and Care Planning for Resident Choice
  • Providing education: results and recommendations

Description:

When performing an assessment of the patient with dysphagia, a chart review is critical to collect relevant information and identify red flags. By knowing how disorders progress and impact each phase of swallow, patient deficits can be better understood. Also, there are certain evaluation tools which provide clinicians the details needed to document a deficit. Furthermore, COVID-19 has changed the process of evaluations as clinicians try to minimize risks to their patients and themselves. Some screens are aerosol generating while others are not. Therefore, consideration must be given into what may be aerosol generating in the evaluation. As always, the clinician's job is to educate the patient and family about dysphagia as well as any recommendations for diet/liquid modification, swallow strategies, and the importance of compliance. When a patient and/or family refuses to accept the results, waivers are not the best option. Utilizing EDAR and Care Planning for Resident Choice provides patients with choices when they opt out of treatment recommendations.
